Ms. Avalos not only explained what we needed to do for my wife's name change on her birth certificate, but also made coordination with the newspaper publishing required for our court order of name change. She communicated to keep us informed before and after the name change court hearing. She prepared all necessary forms for our Vital Record Department application to make the amendment to her birth certificate for the name change to take effect.
